B-series Cummins cylinders at 448k miles

The cylinders look fine to me, why did the head go? If the UOAs are fine and if oil pressure is within spec, I wouldn't do bearings or the oil pump. Who's supplying the head or will be an Cummins reman?

I don't know much about diesels - but those Cummins B/ISBs are good engines. Cummins had the recipe down for them.

Possible increased wear from being at a lower temperature. There was discussion on cool temps on cylinder liners increasing wear or something to that effect
 
How well is air distributed among the cylinders? How about EGR? In some cases, some cylinders have less air than others and it results in an uneven fuel air mixture.

I know this is a far bigger problem on inline 6 gasoline engines, but I guess it could happen to a diesel.

No EGR on this engine model. The 6B has the intake plenum cast integrally with the head, and it does taper to a smaller volume back at cylinder 6, so I would expect maybe less air back there.
