Yes, if 20w50 didn't quiet it then I doubt it is engine internals as 20w50 would quiet both of my Brothers accords quite nicely. OR your engine internals are SO bad that nothing would help but I doubt it. I have never heard of anyone haveing a bad rod on a Honda engine ever. Usually the body rots out before the engine goes kaput. Might be time to pull the oil pan as others have suggested. Not piston slap on 1 piston.

If it is a rod then you have a dilema. Sell the car or return it and the in-law gets steamed because YOU made them look stupid when it came to choosing a vehicle. So they constantly complain to your wife about how you are "Mr Know it ALL" with cars and that engine was fine before you broke it. Now your daughter is upset because her new wheels are broken by you as well and she wants to go visit her friends in it and you keep taking it apart looking for some problem when it "runs fine". My best advice is quietly get it fixed at any cost and never say a word to anyone or all of this will be your fault and you'll end up sleeping on the couch and your teenage daughter won't speak to you for a month.

I have a 2001 Civic that has been religiously maintained. It developed this very noise at about 67k. I now have 139k on it and it's the same. Knocks when cold, goes mostly if not all the way when hot. Valves are adjusted properly.

Same here. The ex's Civic made the exact same noise for 80K and was still going strong last I heard.

If it's a rod, it will get worse as the engine heats up. If it stays the same or gets better with heat, it's not a rod. Normally, if you have to wonder if it's a rod or not, it's probably not.
 
I've got a '95 Accord with the F22B1 (VTEC version of your engine). It's a little noisy at idle, and every F22 I've ever heard in person makes the same noise.

Yours is far worse than any I've heard yet. If my car sounded like that I'd be worried.
 
I have 5 honda/acura vehicles and this is normal "loose valve train" sound. Some are worse than others. Valve adjustment may help. I don't think anything is wrong and would not attack the lower engine.

Sounds like more of a clack-clack sound than a knock. Like TexasHonda, I think it's valve train noise. Apples and oranges; but my Legend makes the same noise. It's had this noise when I got the car from the original owner at 270k miles....now at 390k miles (engine has never been apart) I'm not going to worry about it.

Do Honda 4-cyl. engines use hydralic adjusters? My C27A uses them and they tend to varnish up and the valve train gets "ticky". +100k miles running Delo 15w40 has cleaned them up enough so that they only only tick for 15 seconds or so on cold start-up before they pump back up.

Yeah, I've considered AutoRx :)
