Yes, the 5s still works here in the USA. My Dad still uses his on a daily basis with no complaints.

Its not even a thought to think about. It would be like wondering if you should buy a phone today because your not sure if the service life will be long enough.
The Service life will be good for just as long any new phone that you buy today.
