Toyota Strut Mount & Bearing clarification

Why are you replacing the strut mounts?

Even on the high mileage Toyota products, I only replace the boot. In my experience, the bearing and mounts rarely fail.
 
The red curved rubber should go up.

The mount may be on its way out - when I turn left there is this thud-thud sound coming from the mount.
The struts have 40K on them or even less and the sound is quite annoying.
Toyota tech once told me that its best to replace the mounts when replacing the struts incl. the boots.
Well atleast in the case of my vehicle he was right .. what a PITA job. When I was 24 it was a snap, now at 44 its like man should I have to go under for this mount.
