NTSB cites autothrottle & crew in Asiana 777 crash

Failure to fly the airplane. You don't start messing with the autopilot and automation on short final.
 
Bottom line, if you're not stabilized and steady on the approach by about 5 miles out, you do a go-around and try again. Period. No debate, pilots are told this. Now, it embarasses some of them, so they get persistent with an unsteady approach, which increases risk dramatically.
